Kratom Withdrawal & 7-OH Addiction: What to Know
Kratom Withdrawal Is Sending More People to Addiction Treatment: What to Know
Addiction treatment providers are reporting more people seeking help for dependence and withdrawal involving kratom and concentrated 7-hydroxymitragynine, commonly called 7-OH.
Traditional botanical kratom and concentrated 7-OH products are not necessarily the same exposure. Understanding that difference can help individuals and families recognize withdrawal symptoms, dependence and when professional treatment may be appropriate.
Why Kratom Withdrawal Is Getting More Attention
A September 2026 report from STAT described addiction treatment programs seeing a substantial increase in patients seeking help for kratom and concentrated 7-OH use. At one treatment provider cited in the report, the percentage of new patients reporting kratom use increased from approximately 3% in September 2025 to 14% in August 2026.
Why Are More People Seeking Treatment for Kratom?
Kratom comes from Mitragyna speciosa, a tree native to Southeast Asia. Its leaves contain several naturally occurring alkaloids, including mitragynine and relatively small amounts of 7-hydroxymitragynine.
Traditional products have commonly included powders, capsules and teas. More recently, consumers have encountered concentrated extracts, tablets, gummies, liquid shots and other products containing much higher levels of 7-OH than occur naturally in the plant.
Traditional Kratom
Botanical kratom is generally made from kratom leaves and may be sold as powder, tea or capsules.
Concentrated Extracts
Extracts may contain much greater concentrations of kratom alkaloids than traditional leaf products.
7-OH Products
Some newer products contain enhanced, concentrated or synthetic 7-OH with substantially different potency than botanical kratom.
Botanical Kratom and Concentrated 7-OH Are Not Identical
Federal regulators have specifically focused on highly concentrated or enhanced 7-OH products. Someone using traditional leaf kratom may have a different exposure than someone taking high-potency 7-OH tablets or extracts.
Kratom vs. 7-OH: Why the Difference Matters
Marketing can blur the distinction between traditional kratom and concentrated 7-OH products. Understanding the difference can help explain why withdrawal experiences may vary significantly.
| Feature | Traditional Botanical Kratom | Concentrated 7-OH Products |
|---|---|---|
| Source | Kratom leaf from Mitragyna speciosa | Products containing enhanced or concentrated 7-OH |
| Common Forms | Powder, tea and capsules | Tablets, gummies, shots, powders and dissolvable products |
| 7-OH Level | Occurs naturally in relatively small amounts | May contain substantially higher concentrations |
| Main Concern | Dependence and other adverse effects can occur | Greater concern about potent opioid-like effects, tolerance, dependence and withdrawal |
| 2026 Federal Focus | Not the primary target of the DEA concentrated 7-OH action | DEA initiated temporary scheduling procedures for certain concentrated 7-OH substances |
Federal Action Is Focused on Concentrated Products
In July 2026, the DEA announced a temporary scheduling process involving 7-OH above a specified threshold and several related substances. The agency distinguished these products from botanical kratom containing naturally occurring trace quantities of 7-OH.
What Does Kratom Withdrawal Feel Like?
Kratom withdrawal varies from person to person. Symptoms may depend on the product being used, potency, amount, frequency, duration of use, other substances and individual health.
Physical Symptoms
- Sweating or chills
- Muscle or body aches
- Fatigue
- Nausea
- Diarrhea or stomach discomfort
- Reduced appetite
- Difficulty sleeping
Emotional & Behavioral Symptoms
- Anxiety
- Restlessness
- Irritability
- Agitation
- Low mood
- Difficulty concentrating
- Cravings
The FDA has identified restlessness, body aches, fatigue, irritability and cold sweats among withdrawal symptoms associated with 7-OH products.
How Kratom Dependence Can Develop
Physical dependence occurs when the body adapts to repeated exposure to a substance. When use is reduced or stopped, withdrawal symptoms may emerge.
Tolerance may also develop, meaning a person needs more of a product or needs to use it more frequently to experience the effect they previously experienced at a lower amount.
Regular Use
Repeated exposure gives the body time to adapt to the substance.
Tolerance
The same amount may no longer produce the same effect, which can lead to increasing use.
Physical Dependence
The body adapts enough that reducing or stopping use can produce withdrawal symptoms.
Dependence Is Not Automatically Addiction
Physical dependence alone does not establish a substance use disorder. Addiction involves a broader pattern that can include impaired control, cravings and continued use despite significant consequences.
Signs Kratom or 7-OH Use May Be Becoming a Problem
Problematic use can develop gradually. Changes in dose, frequency, withdrawal symptoms and control over use may become noticeable before someone identifies the pattern as addiction.
Increasing Amounts
Needing larger amounts or moving to stronger extracts may indicate increasing tolerance.
Frequent Daily Use
Using several times throughout the day or soon after waking may indicate physical dependence.
Unable to Cut Back
Repeatedly trying to reduce or stop use without success can indicate that professional support may be appropriate.
Withdrawal Between Doses
Anxiety, sweating, body aches, restlessness or stomach symptoms when a dose is delayed may indicate dependence.
Hiding Use
Concealing the amount used, how often it is used or how much money is being spent can be concerning.
Continued Use Despite Harm
Continuing use despite health, financial, work or relationship problems may indicate a substance use disorder.
Can You Detox From Kratom at Home?
Some people discontinue lower-intensity kratom use without formal withdrawal management. That does not mean at-home withdrawal is appropriate for everyone.
The safest approach depends on what product is being used, how much is used, how often it is taken, how long use has continued, other substance use, medical history, mental health and previous withdrawal experiences.
Medical Assessment
A clinician can consider the product, pattern of use, withdrawal symptoms and other health factors before recommending next steps.
Explore Drug Withdrawal ManagementIndividualized Support
There is no single kratom or 7-OH withdrawal plan that is appropriate for every person.
Explore Medical DetoxSeek Emergency Care for Severe Symptoms
Difficulty breathing, seizures, loss of consciousness, severe confusion, chest pain, extreme agitation or another medical emergency require immediate evaluation. Call 911 when emergency symptoms are present.
How Is Kratom Withdrawal Treated?
There is currently no FDA-approved medication specifically for kratom withdrawal. Treatment therefore needs to be individualized rather than based on a universal kratom detox protocol.
A clinical assessment may evaluate the exact products being used, frequency and amount, withdrawal symptoms, other substance use, physical health, mental health and previous treatment history.
Medical Monitoring
Clinical monitoring may be appropriate when withdrawal symptoms, concentrated products or other substances complicate the situation.
Symptom Management
Supportive treatment may be used to address specific withdrawal symptoms based on individual clinical needs.
Behavioral Health Care
Therapy and mental health support can address cravings, anxiety, triggers and the reasons a person began using kratom.
What Happens After Kratom Detox?
Withdrawal management addresses physical stabilization. If kratom or 7-OH use has developed into a substance use disorder, detox alone may not address the behavioral, emotional or environmental factors contributing to continued use.
Some people initially use kratom for pain, anxiety, depression, insomnia, opioid cravings or withdrawal from another substance. Those underlying concerns may still need treatment after kratom use stops.
Residential Treatment
Residential treatment can provide structured support, therapy, recovery education and continued clinical care after withdrawal stabilization.
Explore Drug RehabContinued Addiction Treatment
PHP, IOP, outpatient care, dual diagnosis treatment and continuing support may be appropriate depending on individual needs.
Explore Treatment ProgramsWhen Should You Ask for Help?
You do not have to wait until kratom or 7-OH use becomes an emergency before asking for help.
Consider speaking with an addiction treatment professional if you are experiencing persistent withdrawal, escalating use, repeated unsuccessful attempts to stop, frequent use of concentrated 7-OH products, polysubstance use or continued use primarily to prevent withdrawal symptoms.
A Professional Assessment Can Clarify the Next Step
An assessment can help determine whether medically supported withdrawal, residential treatment, outpatient care or another level of treatment may be appropriate.

FAQ: Kratom Withdrawal, 7-OH & Treatment
Can kratom cause withdrawal?
Yes. Regular kratom use can result in physical dependence, and reducing or stopping use after dependence develops may cause withdrawal symptoms. Severity varies according to product potency, amount, frequency, duration of use and individual factors.
What are common kratom withdrawal symptoms?
Reported symptoms include anxiety, restlessness, irritability, difficulty sleeping, sweating, body aches, fatigue, gastrointestinal discomfort, reduced appetite and cravings.
Is 7-OH the same as kratom?
Not exactly. 7-hydroxymitragynine occurs naturally in kratom in relatively small amounts. Some commercial products contain enhanced, concentrated or synthetic 7-OH at substantially higher levels.
Can you become dependent on 7-OH?
Yes. Concentrated 7-OH has potent opioid-like activity and has been associated with tolerance, physical dependence, withdrawal and addiction-related concerns.
How long does kratom withdrawal last?
There is no universal kratom withdrawal timeline. Duration and severity depend on the product, potency, amount, frequency and duration of use, individual health and whether other substances are involved.
Is kratom withdrawal dangerous?
Withdrawal experiences vary. Some people experience manageable symptoms while others may need medical support. Concentrated products, underlying health conditions and other substance use can make withdrawal more complicated.
Is there medication for kratom withdrawal?
There is no FDA-approved medication specifically for kratom withdrawal. Clinicians may use supportive medications or, in selected circumstances, treatments also used for opioid use disorder after an individualized medical evaluation.
Should I go to detox for kratom?
The appropriate level of care depends on the product being used, level of dependence, withdrawal severity, medical and mental health history, other substance use and previous attempts to stop.
Why are treatment centers seeing more kratom cases?
Addiction treatment providers have reported increasing numbers of people seeking help for kratom and concentrated 7-OH use. Newer high-potency products may contribute to different dependence and withdrawal patterns for some users.
Where can someone get help for kratom dependence?
An addiction treatment professional can evaluate withdrawal symptoms, substance use patterns, medical concerns, mental health and other factors to determine an appropriate level of care.

I woke up many mornings feeling safe, rested, and genuinely hopeful about my future—something I didn’t think was possible before coming here.
At the same time, I was encouraged to do the hard work. The staff challenged me—in the best way—to start building healthy habits and practicing behaviors that support long-term sobriety. They consistently met me where I was, while also giving me the respectful space I needed to approach difficult topics at my own pace.
This balance made all the difference.
Five stars. I leave here feeling capable, confident, and equipped for my sober future.
